Evergreen State College has moved to a paperless medical eligibility process. Every athlete will be required to complete the physical packet below before they are cleared for participation. Physicals will be held on Evergreen State College campus prior to the first day of practices. An email will be sent to all student athletes on record from athletic training staff to their Evergreen email about dates and times of physicals.
